A publicly accessible repository that documents reported incidents where AI-driven systems have caused harm or produced negative outcomes for individuals, communities, or society. Major databases include AIAAIC (AI, Algorithmic, and Automation Incidents and Controversies), the AI Incident Database (AIID), the OECD AI Incident Monitor, and the Database of AI Litigation (DAIL). These databases serve as watchdog mechanisms in the absence of comprehensive AI regulation, tracking details such as the AI system involved, the harm caused, affected populations, and any remedial actions taken.
